What to Bring:
Bring a photo ID, your preferred payment type (cash, credit/debit or money order) and an open/positive attitude. That’s it.
PROGRAM ATTENDEE FORMS:
In a rush? Print and complete the following three forms and bring them with you to your appointment. No printer? No biggie...We will take care of these when you arrive at your scheduled appointment time.
