Our Services

OWI Evaluation (ZOOM)

$125

OWI Substance Use Evaluations

Iowa Code 321J.22 requires individuals convicted of Operating While Intoxicated (OWI) to complete a substance use disorder screening as part of the criminal court proceeding. This evaluation is also required by the Iowa DOT's driver's license reinstatement process. Our licensed clinicians provide evaluations that satisfy this requirement while also offering a comprehensive assessment of your individual circumstances and substance use history.

We believe that an evaluation should be more than a regulatory requirement. Our goal is to provide an objective, evidence-based assessment that considers the whole person and results in recommendations that are appropriate to your situation.

We work closely with clients, attorneys, referral agencies, and the Iowa Department of Transportation to ensure that all required documentation is completed accurately and submitted in accordance with state requirements. Following your evaluation, all necessary reports and recommendations will be provided to the appropriate parties as authorized.

Substance Use Disorder Assessment (ZOOM)

Licensed Substance Use Disorder Assessments

If you have been referred by an attorney, court, employer, licensing board, Department of Transportation, or another agency, our licensed clinicians provide comprehensive Substance Use Disorder (SUD) Assessments that meet applicable state and referral requirements.

Our assessments go beyond simply checking a box. We take the time to understand your unique circumstances, review relevant history, and provide an objective, evidence-based evaluation. Whether your assessment is required for legal, administrative, employment, educational, or personal reasons, we will guide you through the process from start to finish.

We work closely with clients, attorneys, referral sources, and government agencies to ensure that all required documentation is completed accurately and submitted in a timely manner. Following your assessment, you will receive recommendations and reporting that comply with applicable regulations and referral requirements.

This assessment is appropriate for:

  • Non-DOT driver's license reinstatement requirements, including assessments required for driving while barred or other non-OWI driving-related matters
  • Court-ordered substance use evaluations related to non-OWI criminal charges
  • Attorney referrals for legal proceedings or case preparation
  • Probation and parole requirements
  • Professional licensing board requirements and disciplinary matters
  • Employment and workplace referrals, including employer-mandated evaluations
  • Educational institution referrals and student conduct matters
  • Personal or family concerns regarding substance use, including situations where an individual or family member would like a professional assessment and recommendations

Our goal is to provide a professional, respectful, and thorough assessment process while helping you satisfy any reporting or documentation requirements associated with your referral.

Convenient Telehealth Appointments

All evaluations are conducted through secure telehealth appointments using Zoom. This allows you to complete your evaluation from the comfort and privacy of your home while receiving the same professional service as an in-person appointment.

You will receive your Zoom meeting invitation by email on the morning of your scheduled appointment. Prior to your appointment, please ensure that the Zoom Workplace app is downloaded and installed on the device you will use for your virtual meeting. A stable internet connection, working camera, and photo identification will be required at the time of your evaluation.
